Abstract   The authors reviewed 34 patients who received pancreataduodenectomy for cancer of the pancreas head portion, ampulia of Vater and distal common bile duct at the Department of Surgery, Keimyung University Dongsan Medical Center from January, 1982 to December, 1989 and the results were summarized as follows: 1) Among 34 cases, 7 cases were pancreas head cancer, 13 cases were ampulla of Vater cancer and 14 cases were distal common bile duct cancer. 2) Twenty eight patients were male, six were female and the mean age was 55.9 years(range:33-69 years). 3) The most common clinical manifestation was jaundice and itching sensation (76%) and the next were abdominal pain snd weight loss. Jaundice was most prominent in distal common bile duct cancer but weight loss was in pancreatic head cancer. 4) Histologically, all cases were adenocarcinomas. 5) The most common postoperative complication was 7 cases of wound infection with associated anastomotic fistula and intraabdominal abscess. 6) Hyperbilirubinemia (10 mg% or more), usage of stent and type of reconstruction showed no significant relation to the occurence of complications. 7) Two cases were dead within one month after pancreatoduodenectomy and the causes of death were sepsis following leakage of pancreatico-jejunostomy site. No patient survived over 3 years after operation in pancreas head cancer but 5-year survival rate of distal common bile duct cancer and ampulla of Vater cancer were 22.6% and 29.2%, respectively.
